Replacing a Lost or Stolen Passport
American citizens whose passport has been lost or stolen should file a police report with the Indian authorities as soon as possible. If you think you know where your passport was lost or stolen (in a hotel, airport, railway station, etc.) you should report the loss at that location as well. Once you have done this, come to the Embassy during working hours with the following items:
- Police report
- Proof of your identity and U.S. citizenship, if available.
- Three recent, identical 2"x2" color photographs taken against a white background. No other format can be accepted.
- Completed DS-11 Passport Application.
- Completed form DS-64 Statement Regarding Lost or Stolen Passport
- Replacement fee (US$85 for those under 16 years of age, US$100 for those 16 or older). The fee may be paid in cash dollars, cash rupees (at the current exchange rate) or with a major U.S.-based credit card.
Persons who lose their passport with their valid Indian visa must apply for an exit visa before departing the country. More information on obtaining an exit visa will be provided when you come to apply for a replacement passport. Exit visas are not issued on weekends or Indian holidays.
