Adding Visa Pages
The Embassy can add 24 additional visa pages to a U.S. passport. This service is provided free of charge. The passport holder must complete Form DS-4085 "Application for Additional Visa Pages". This service can be done in person in approximately 20 - 30 minutes, or by mail/messenger (using the same procedure as for renewing an adult passport by mail). Note that the passport holder (or the holder's parents in the case of a minor under age 16) must sign the application, even if the form and passport are delivered to the American Citizen Services Unit by another person.
Name Changes and Other Amendments
The Department of State has determined that, for security purposes, the passport data page (the "face page") of each U.S. Citizen's passport should directly contain all important data, including name, sex, date of birth, place of birth, and passport validity information. Accordingly, applicants can no longer request passport amendments to change these data.
If a passport holder determines that any of the information on the passport's face page is incorrect, he/she must apply for a new passport. Similarly, if an applicant changes his/her name (e.g., because of marriage or legal name change), he/she must apply for a new passport. A new passport to correct or change any of the data on the face page of the passport can be provided free of charge if the applicant so requests within one year of the date of issuance of the original passport. To request a free replacement passport to reflect a name change or to change or correct any of the other data contained on the passport's face page, the applicant should complete Form DS-5504 "U.S. Passport Re-Application Form".
If more than one year has passed since the passport was issued, the applicant must follow the normal passport renewal procedures, paying the full renewal fee to obtain a replacement passport.
